Dr. Tilles’ Program
Dr. Tilles has been practicing Internal Medicine, with a subspecialty in Nephrology, for over 30 years. He currently practices in San Jose, California. Dr. Tilles will continue to accept new members until his membership panel maximum of 100 patients is reached.
What to do if you’re interested in becoming a member:
- Using the link below, please complete and submit a Membership Agreement form. After Dr. Tilles countersigns your agreement, an MDprime representative will contact you to confirm your membership and a welcome packet will be sent to you in the mail. - A note about the Privacy Authorization Agreement (attached to the Membership Agreement, pages 7-9) – The Health Insurance Portability and Accountability Act of 1996 (HIPAA), a federal law requiring healthcare providers to safeguard patients’ Protected Health Information (PHI), outlines ways in which healthcare providers can and cannot use PHI. In practice, HIPAA prevents healthcare providers from using unsecured/non-HIPAA-compliant email services (e.g., Gmail, AOL, Yahoo, etc.) to communicate with their patients. This means that as a member of Dr. Tilles’ concierge medicine program, he will not be able to initiate communication with you through these email services. By filling out the Privacy Authorization Agreement, you acknowledge and agree: (1) that using non-HIPAA-compliant email services is an unsecured method of communication; (2) that your PHI might be at risk of interception by using these services; and (3) if you would still like to initiate communication with your doctor using these email services, in spite of the foregoing information, you do so on your own accord.
